We start where the money leaks.

Most software projects begin with a requirements document. Ours begin with a question: which part of this business costs the most to run by hand? That answer is rarely the thing on the roadmap, and it is almost always the thing worth building first.

So we sit with the people doing the work. We watch the spreadsheet get copied into the other spreadsheet. We count the hours. Then we write down a scope, a number, and what will be different when we're done, before anyone opens an editor.
